Use this calculator for the common dosing sketch AjBW = IBW + 0.4 × (actual weight − IBW). Ideal body weight uses the Devine formula: 50 kg + 2.3 kg per inch over 5 ft for men, 45.5 kg + 2.3 kg per inch over 5 ft for women. Confirm with the protocol you actually follow; some aminoglycoside sketches use AjBW when BMI is high.
Height can be entered in centimetres (cm) or inches (in). Actual weight, IBW, and AjBW can be shown in kilograms (kg) or pounds (lb).
This is not a prescription. IBW can be unrealistic at very short heights because Devine is linear from 60 inches.
Sex: Chooses the Devine intercept (50 vs 45.5 kg).
Height: Standing height, in cm or in.
