Use this calculator to add the HAS-BLED items: uncontrolled hypertension, renal disease, liver disease, prior stroke, bleeding history or predisposition, labile INR, age over 65, drugs (antiplatelet or NSAID), and excess alcohol. Each yes is 1 point; the maximum is 9.
A high score flags caution; it does not forbid therapy by itself. Confirm the definitions your protocol uses for renal and liver disease.
Hypertension: Uncontrolled high blood pressure.
Renal disease: Dialysis, transplant, or creatinine around 2.3 mg/dl or higher (definition varies).
Liver disease: Cirrhosis or a significant enzyme rise.
Prior stroke: Previous stroke.
Bleeding history: Prior major bleed or a predisposition to bleed.
Labile INR: Unstable INRs if on warfarin.
Age over 65: Older than 65 years.
Drugs: Antiplatelet or NSAID.
Alcohol: Excess alcohol.
Score: Sum of the nine items.
