About this calculator

Use this calculator to add one point each for respiratory rate 22 or more, new confusion or a GCS drop, and systolic blood pressure 100 mmHg or under. A score of 2 or 3 is the usual prompt to look harder for infection and organ failure. Full SOFA needs PaO2, platelets, bilirubin, GCS, MAP, and creatinine; this page keeps the quick screen. qSOFA is not a definition of septic shock.

RR ≥22: Respiratory rate 22 breaths/min or more.

Altered mentation: New confusion or GCS drop.

SBP ≤100: Systolic 100 mmHg or under.

Score: Sum of the three flags, 0–3.
