Use this calculator for Devine IBW: men 50 + 2.3 × (height_in − 60) kg, women 45.5 + 2.3 × (height_in − 60) kg, then tidal volume = IBW × ml/kg. Six ml/kg is the ARDSNet lung-protective sketch; 8 ml/kg is a common start. Height under 5 feet still uses the same line and can yield a low IBW. This is not spirometry and not an ETT-size page.
